17. Take the Eraser tool with these settings:

Follow the screenshots to see what to erase on each layer. Here we go:

W1 layer:

almost everything. 

W2 layer:

only 1/3 is left.

W3 layer:

only1/2 is left.

W4 layer:

just a litle erased.

W5 layer:

nothing erased

Close all layer but keep the P1 layer visible.
Make the top layer active but keep it closed. This is how you palette should look like:

 

18. Layer new raster layer and name it N1. Set Background Color to #6F869F and Foreground color to #48596B. Take your text tool and type in the name you want , Antialias checked floating checked and size 48 font I used is Bellbottom. Set your name on the right place. Keep the selection.

19. Selection Defloat you still have marching ants.
Duplicate N1 , 4 times and rename those layers N2 , N3, N4 and N5.
Close layers N2 ,N3, N4 and N5 ( ALL OTHER LAYERS ARE STILL CLOSED TOO ONLY P1 IS VISIBLE) be sure that N1 layer is the active layer .

Selections --> Modify --> Expand 1 pixel.

On  N1 layer. Go to Effects --> Plugins --> Almathera A puddle and use these settings: Reflection - 175 ,Displacement - 75 ,Lighting strength - 5 ,Perspective-5 and Scale - 50.
Close N1 layer and make N2 layer active. Go to Effects --> Plugins --> Almathera A puddle and use these settings: Reflection-180 the rest will be the same as above.
Close N2 layer and make N3 layer active. Go to Effects --> Plugins --> Almathera A puddle and use these settings: Reflection-185 the rest will be the same as above.
Close N3 layer and make N4 layer active. Go to Effects --> Plugins --> Almathera A puddle and use these settings: Reflection-195 the rest will be the same as above.
Close N4 and make N5 the active layer. Select None.
Go to Effects /3D Effects / Inner Bevel with preset pillow but change the Elevation to 30. Go to Effects / 3D Effects / Drop Shadow with these settings:

 20. Make P1 the active layer. Layer new layer. Layers / Arrange/ Send to bottom. Floodfill this layer with white #FFFFFF or a color of your choice. Duplicate this layer 5 times and rename these 5 layers to B1, B2, B3, B4 , B5 and B6 . Close all layers by clicking on the glasses.
